Condo Act Forms for Owners

Please note that the following are prescribed forms created by the Government of Ontario (all of the forms were updated by the government on May 11, 2018). The forms can be opened directly from Internet Explorer – if using another browser they need to be saved to your computer first (right-click to save) and then opened with Adobe Reader directly from your computer. The Request for Records form prescribed by the Condo Act must be used as per Reg. 48/01: s. 13.3 (3).
